Mephallo can be found on the ghostly bridge located in the Shivering Step.
To make the bridge appear you must first collect the Guide to Sight through fishing in Shivering Step.
After you inspect the book head to the large gargoyle statue or where the Vitheo statue is on Stowaways Steps.
A large ghostly bridge should now be there where you can cross and battle Mephallo.
